Service Coordinator-Community Care 
Based at Hurstville, New South Wales

Quality Health Care is a specialist in-home, disability and community care provider, providing support and services throughout Sydney. Our services are individually tailored to meet the needs of each client and provided in an environment that is caring, friendly and supportive.

 

We are seeking a passionate coordinator, who has excellent customer service skills and enjoys providing high-quality in-home care,  supporting older people and people with a disability living independently at home. We provide tailored support to meet the different needs and preferences of our clients. The successful applicant will be positive and engaging and will liaise with clients, providers and QHC staff on a daily basis to ensure a reliable and consistent service. You will be part of a team that is responsible for providing high quality support that builds capacity and enables people to maintain their independence at home.


Your Role

  • Coordination and scheduling of client support plans and rosters

  • Scheduling with consideration of staff availability, service user requests, and training requirements

  • Liaising with staff, service users and management

  • Adjusting rosters to accommodate ongoing changes

 

Essential Criteria

  • Strong customer service skills

  • Attention to detail with the ability to match staff to individual clients with consideration of; skills, personality and cultural preferences

  • Ability to problem solve, mediate and find creative solutions to issues as they arise

  • Answering new client service inquiries in a knowledgeable and supportive manner

  • Good verbal and written communication

  • Demonstrated ability to multitask and prioritise work

  • Professional and confident phone manner

  • The ability to adapt to varying work demands and to be flexible when required

  • Own vehicle would be an advantage
