Support Coordinator

We are looking for a passionate and enthusiastic team member who wants to make a difference in the lives of people living with a disability or mental illness. With excellent communication and interpersonal skills you will have the ability to engage with clients, building relationships and liaising with families and carers. With a sound understanding of the NDIS and community services sector, you will provide a tailored service in keeping with the needs and goals of your clients.

 

The role as a Support Coordinator includes facilitating effective working relationships and communications with people with disability, carers, families and other key stakeholders to support the implementation of NDIS plans and capacity building.

 

  • Work closely with the health system and local providers to source reputable and effective supports and services to enable participants to reach their goals and gain greater independence to engage in all areas of their lives.

  • Research, coordinate and manage a range of varied levels of specialist supports across multiple providers, to suit participant's complex needs

  • Use a strength-based and person-centered approach to build skills capacity of participants, ensuring their goals & choices drive the process of engaging supports

  • Strengthen and enhance the participant’s abilities to self-direct their own supports and participate in the community

  • Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports

  • Support the participant through identifying and selecting providers, monitoring plan outcomes & plan expenditure

  • Coordinate assessments, reports and service proposals

 

Essential Criteria

  • Allied Health qualifications; or minimum Certificate IV in Disability or similar; or relevant practical experience

  • Experience in providing services to NDIS participants as a Support Coordinator

  • Experience in reading, understanding, and implementing NDIS plans

  • Experience in connecting clients to services under a client direct care model

  • Excellent interpersonal and communication skills and a strong commitment to customer service

  • Previous experience in a billable environment highly desirable

  • Strong customer service skills with demonstrated ability to develop and maintain relationships with people with disability and/ or their families and support network and service providers

  • Resilience and competence in high level interpersonal communication, negotiation and conflict resolution skills

  • Sound time management skills and demonstrated ability to work with minimal supervision

  • Demonstrated ability to make complex judgements, prioritise tasks and take initiative

  • Strong written communication skills with the ability to write high quality reports

  • Current driver's license and willingness to travel to clients where required
