Support Coordinator-NDIS 
Based at Hurstville, New South Wales

Quality Health Care is a specialist in-home, disability and community care provider, providing support and services throughout Sydney. Our services are individually tailored to meet the needs of each client and provided in an environment that is caring, friendly and supportive. As a result of expansion we are seeking experienced Support Coordinators for our South West team. Working with participants, families, and providers our focus is to offer high quality support that delivers positive results. Working closely with the health system and local providers you will source reputable and effective supports and services to enable participants to reach their goals in a more complex environment of gaining greater independence and capacity to engage in all areas of their lives.

Your Role

  • Research, coordinate and manage a range of varied levels of specialist supports across multiple providers, to suit participant's complex needs

  • Use a strength-based and person-centered approach to build skills capacity of participants, ensuring their goals & choices drive the process of engaging supports

  • Strengthen and enhance the participant’s abilities to self-direct their own supports and participate in the community

  • Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports

  • Support the participant through identifying and selecting providers, monitoring plan outcomes & plan expenditure

  • Coordinate assessments, reports and service proposals


Essential Criteria

  • Allied Health qualifications; or minimum Certificate IV in Disability or similar; or relevant practical experience

  • Experience in providing services to NDIS participants as a Support Coordinator

  • Experience in reading, understanding, and implementing NDIS plans

  • Experience in connecting clients to services under a client direct care model

  • Excellent interpersonal and communication skills and a strong commitment to customer service

  • Previous experience in a billable environment highly desirable

  • Strong customer service skills with demonstrated ability to develop and maintain relationships with people with disability and/ or their families and support network and service providers

  • Resilience and competence in high level interpersonal communication, negotiation and conflict resolution skills

  • Sound time management skills and demonstrated ability to work with minimal supervision

  • Demonstrated ability to make complex judgements, prioritise tasks and take initiative

  • Strong written communication skills with the ability to write high quality reports

  • Current driver's license and willingness to travel to clients where required
