Team Leader - Supported Independent Living
Based in Merrylands, New South Wales

Quality Health Care is a specialist in-home, disability and community care provider, providing support and services throughout Sydney. Our services are individually tailored to meet the needs of each client and provided in an environment that is caring, friendly and supportive. We are seeking an experienced and skilled Team Leader with a strong Disability/Nursing background to lead a team of QHC staff and to oversee a Specialist Disability Accommodation accommodating people with high physical support needs, health needs and psycho-social needs based in Merrylands.  


The model supports 10 residents in a range of 1 and 2 bedroom units and you will lead a team of Support Workers to ensure the effective management and operation of the services and ensure that residents receive targeted, flexible and customised support that increases skills and independence. Our preferred candidate will have strong people skills, a strong practice focus and enjoy providing high-quality support that builds capacity to enable independent living.

Your role

  • Ensure NDIS plans, goals and activities are supported and progressed over the lifetime of the plan

  • Complete service rosters in line with resident support needs and NDIS funding allocations

  • Ensure staff are fulfilling their duties and responsibilities and maintain high standards in relation to practice, services and relationships

  • Build rapport with residents, families and carers and ensure channels of communication are open, positive and productive


Essential Criteria

  • Appropriate Degree (health science) incl OT, Nursing, Social Work, Psychology

  • Extensive experience working within a residential setting and/or working with people with a physical disability and/or mental health

  • The ability to develop, implement, monitor and review Person Centred Plans, Individual Support Plans and Behaviour Support Plans using an individualised planning approach

  • The ability to think outside the square and connect with others (residents, staffing peers and management)

  • Experience in leading and managing teams

  • Well organised, analytical with good time management skills

  • Strong teamwork and interpersonal skills

  • Current, unrestricted Class C NSW drivers licence
