Team Leader - SIL
Based at Mortdale, New South Wales

Quality Health Care is a specialist in-home, disability and community care provider, providing support and services throughout Sydney. Our services are individually tailored to meet the needs of each client and provided in an environment that is caring, friendly and supportive.


We are seeking an experienced and passionate Team Leader with a strong Disability and Mental Health background to lead a team of QHC staff and to oversee support services for residents at an 8 bed model in East Sydney. You will be experienced in working with people who have a dual diagnosis and require ongoing complex support. Our preferred candidate will have strong people skills, a strong practice focus and enjoy providing high-quality support that builds capacity to enable independent living.


Your role

  • Identify residents' goals, needs, strengths and aspirations using a person-centred framework embodied by QHC

  • Provide informal, and where required formal, peer support and supervision to develop team members

  • Work collaboratively with residents, their families and natural supports, providing information, education and skills training as appropriate

  • Participate in the establishment, development, support and maintenance of links and partnerships between the mental health service, other government and health services, NGOs and the community sector.


Essential Criteria

  • Extensive experience working within a residential setting and/or working with people with a disability and/or mental health

  • The ability to develop, implement, monitor and review Person Centred Plans, Individual Support Plans and Behaviour Support Plans using an individualised planning approach

  • The ability to think outside the square and connect with others (residents, staffing peers and management)

  • Appropriate Degree (health science) incl OT, Nursing, Social Work, Psychology

  • Excellent verbal and written communication skills

  • Experience in managing staffing teams

  • Current, unrestricted Class C NSW drivers licence
