Home and Community Support
From everyday activities in the home to staying active, we’re here to help with services and referrals. We’re committed to helping you stay independent, in your home for longer and get out and about in the community as often as you can. We offer the following supports and services;
Personal Care
Our team can support you with tasks such as bathing, showering, dressing, grooming and toileting. Our team is sensitive to your needs and will provide support in a respectful way to ensure that you are ready for your day or evening.
Domestic Assistance
We can assist you to keep up with your household chores such as cleaning, laundry, vacuuming and ironing. We can also help with cooking and gardening and support you with chores that are now too difficult for you to manage.
Social Support
We can support you to maintain your social and recreational activities by organizing transport, accompanying you on activities, or providing support as required. We can also provide regular companionship over a cup of coffee or tea, a short stroll or playing cards.
Health
Maintaining your health can sometimes be tricky, maintaining your diet, looking after your wellbeing, or managing your physical needs can be challenging. Our team of healthcare professionals can assist you with a range of supports such as nutrition, rehabilitation, respiratory care, falls prevention and dietary needs.
Shopping
Sometimes it can be difficult getting to and from the shops and we can assist you with this by accompanying you to get the items you need or completing your shopping for you.
Overnight Care
If you need assistance over night to manage an injury or condition our trained staff can stay with you to provide assistance and reassurance. We will remain overnight and when not providing support our staff can be getting on with household chores such as ironing.
Our home and community team works across a couple of different programs, including NDIS, Private Care and Support and we also offered brokered services through the Commonwealth Home Support Program.
NDIS SUPPORT
QHC’s home and community team offers individualised support that enables you to maintain your social and recreational activities by organising transport, accompanying you on activities or providing support as required. Our team will support you to take part in the activities you enjoy and experience new activities that you have always wanted to try. Some of the supports we provide now, include;

Weekly swimming activities and lunch at the local pool

Weekend support to play in a local basketball competition

Weekend fishing at local wharfs with a fish and chip supper at the end of the day

Trips to the cinema to see the latest blockbuster releases
Our team is experienced, trustworthy and diverse. We work closely with you to get the right support match. We ensure our team has the right qualifications, experience and attitude to support you in the right way and we only select staff who are friendly, flexible and trustworthy.
We work with you, to find the right match for you and all staff receive ongoing training. All of our team loves what they do with many team members working at QHC for many years because of the support they receive and the variety of the work. We take pride in the quality of service we provide, and we will always listen to your feedback and ideas on how we can improve.
BROKERED SUPPORT
Our home and community team provides brokered supports through providers such as the Benevolent Society and St Vincent’s de Paul. Our brokered services are provided with privacy and dignity with the goal of assisting people with daily living self-care tasks.
Our team can offer a range of supports and services that will enable you to maintain your independence and remain at home. The supports we provide include;

Transport and helping you get out into the community

Support around the home if you have mobility issues

Monitoring or prompting self-administration of medicine.
Our team is experienced in working with older Australians and we are skilled at providing support that enables you to maintain your independence at home. We understand each person’s needs and daily routines are different and therefore we tailor our care and assistance services to meet your needs. Our team is experienced, trustworthy and reliable and we work with you to get the right match and/or team to support you.
PRIVATE SUPPORT
Our home and community team also support people privately in the community. We can provide 24/7 support or work with you to develop a package that fits in with support as you need it. We employ our own team who can be available to provide support on short notice. Our service provides you with the assurance and peace of mind you are looking for.
We work with you to develop a package of supports that are tailored to meet your needs and supports are flexible and customised to your needs, goals and outcomes. We’re committed to helping you stay independent, in your home for longer and our experienced team are friendly, flexible and provide you with the highest quality of support.
At home and in the community, we provide one-on-one support, seven days a week. You might want:
