Responsibilities
Coordination
Ensure all support coordination intake is referred through and completed via the appropriate channels and procedures in a timely and effective manner.
Coordination of supports of referred clients as set out by NDIS guidelines
Engaging and coordinating service providers on behalf of participants and linking participants to providers.
Meeting with and engaging participant to develop and refine their goals.
Engage participants informal supports (family, carers, etc)
Working with participants to help build their own independence in line with their NDIS plans
Using a coaching approach to assist participants in line with their NDIS plan
Maintaining accurate and up to date case notes on participants progress
Ensuring work health and safety guidelines are upheld and adhered to
Building Knowledge of local services and organisations participants can utilise
Engaging with NDIS staff on behalf of the NDIS participant
Providing crisis support to participants
Assessing and managing risk in line with organisational policies and procedures.
Effective complaints handling between participants and service provision.
Continuous improvement
Demonstrated understanding of all relevant external legislation & internal policies and procedures that relate to position and the organisation
Participate in and contribute to quality improvement programs and other facility activities to meet Service. Accreditation standards
Participate and contribute in workplace health and safety activities to ensure a safe work environment for clients, community, staff and visitors.
Qualifications, Skills and Experience
Minimum requirement of Certificate IV in Disability or other relevant qualifications (the relevance will be at the discretion of management).
4 years mental health/ dual diagnosis in direct support role
Current first aid certificate
Well organised, responsible with good time management skills
Good teamwork and interpersonal skills
Good written & verbal communication skills.
Well organised, detailed and structured
Ability to work under pressure
Excellent communication skills, including presentation and report writing skills
Strong teamwork, interpersonal and organisational skills
Mandatory Requirements
Current Unrestricted Australian driver’s license
Current WWC clearance
Current Senior First Aid Certificate or willingness to obtain within three (3) months of commencement
A capacity to meet the physical requirements of the job, including driving and remaining in a seated position to perform tasks
